This is a shoutout of community partners working together. First and foremost, we thank God. We wouldn't be able to do this work without Oak Park Church of God, Northwest Human Services, Union Gospel Mission, and Northwest Senior Disability!
Greg found himself homeless after losing his housing in 2004. He made his way from Washington to Oregon. Our Outreach team first met him in May 2023 and we started working on goals with Greg in the fall of 2024 at The Wellness Table - Oak Park Church.
Greg has been suffering with arthritis and early dementia, and it was taking a toll on him with the cold winter weather. NWHS Street Outreach was able to meet with him at his camp and build rapport. He stayed consist with his appointments, was referred to Northwest Senior & Disability Services (NWSDS) by NWHS and we got him into Village of Hope.
While he was in our shelter, he worked with our Health Navigators to ensure he was making it to his NWSDS appointments. During this process, he was exited from Village of Hope but our Outreach Team stepped in to continue services.
As the winter was approaching, Greg checked in at the UGM. This made it easier for him to stay connected to all the community partners that were working with him. However, because of physical limitations, it was hard for him to stay engaged, so he made his way back to his camp.
NWHS and Church at the Park Outreach stayed connected with him, so that he could get housed.
This January, he was approved to move into River Park Senior Living in Sheridan. We are happy to announce that he is settling in well! He says he is eating the best he's eaten in a long time, loves his room, and bought himself a 50" tv!
